The above link will take you to a site for a precinct delegate form you can print out. You have to get it notarized before you sign it AND fill in your precinct. If you do not know your precinct, be sure to ask and find out. They will not accept it without that. All parts of the form need to be filled out. You must have your form submitted no later than May 7 turned into the County Clerk’s office.
